Appointment of Attorney and Declaration of Representative
I appoint Tax123 to act as my attorney and agent. This power of attorney operates immediately upon my signing it. My attorney may do one my behalf anything that I may lawfully authorise and attorney to do, subject to the all details set out below part of this document.
a) Gain information regarding the above
b) Contact any of my previous employers to request and receive all the necessary information including the payment summaries, statement of earnings and tax file number.
c) Obtain and providing any information from/to the ATO(Australian Taxation Office), DIAC(Department of Immigration and Citizenship), SHAR(The Superannuation Holding Accounts Reserve) in relation to any benefit to which I may be entitled.
d) Prepare, sign, lodge and process any agreements, consents or other documents (including superannuation applications, transfers, consolidations and claims forms and tax return forms) required to refund any taxes or facilitate the payment(s) of any superannuation benefits;
e) Deposit any tax refund or superannuation fund cheque or money into the attorney’s bank account for the purpose of applying any remaining bills or fees, and deposit any balance of the refund into my bank account.
f) Charge and be paid fees for the work my attorney performs pursuant to this Power of Attorney;
This power of Attorney is effective as today (signed date) until the completion on my taxation and superannuation affairs.
The information provided to Tax123 for the preparation of the above-named documents is true and correct. The tax law imposes heavy penalties for giving false or misleading information.
By signing this declaration, my tax agent to electronically transmit my tax returns via the Electronic Lodgement Service or manually as applicable.
